Antibiotic Medications for NCLEX Pharmacology Review
Antibiotics are among the most frequently prescribed medications and a high-yield NCLEX topic. Understanding antibiotic classifications, mechanisms, adverse effects, and nursing considerations is essential.
Penicillins
Penicillins (amoxicillin, ampicillin, piperacillin) inhibit bacterial cell wall synthesis. They are bactericidal and effective against many gram-positive organisms. Amoxicillin is the most commonly prescribed oral antibiotic. Key nursing considerations include assessing for penicillin allergy before administration (cross-sensitivity exists with cephalosporins in about 1% of cases). Teach patients to complete the full course even if symptoms improve. Monitor for superinfection including oral thrush and diarrhea.
Cephalosporins
Cephalosporins are organized by generations (first through fifth) with increasing gram-negative coverage. First-generation (cefazolin) is commonly used for surgical prophylaxis. Third-generation (ceftriaxone) is used for serious infections including meningitis. Assess for cephalosporin and penicillin allergy history. Avoid alcohol consumption as some cephalosporins cause a disulfiram-like reaction (cefotetan, cefoperazone).
Fluoroquinolones
Fluoroquinolones (ciprofloxacin, levofloxacin) inhibit DNA gyrase. They carry a black box warning for tendon rupture, tendinitis, and peripheral neuropathy. Contraindicated in children and pregnant women due to effects on developing cartilage. Take with water and avoid antacids, dairy, and iron supplements within 2 hours as they decrease absorption. Report any tendon pain or swelling immediately.
Aminoglycosides
Aminoglycosides (gentamicin, tobramycin, amikacin) are used for serious gram-negative infections. They carry significant risk of nephrotoxicity and ototoxicity. Monitor peak and trough levels to maintain therapeutic range while minimizing toxicity. Assess renal function (BUN, creatinine) and hearing throughout therapy. Ensure adequate hydration to protect kidneys.
Vancomycin
Vancomycin is used for MRSA and serious gram-positive infections. Monitor trough levels (target 15-20 mcg/mL for serious infections). Infuse slowly over at least 60 minutes to prevent red man syndrome (flushing, hypotension). Monitor renal function closely.
