Maternity Nursing on the NCLEX
Maternity nursing is a distinct content area on the NCLEX that covers the entire childbearing cycle from conception through the postpartum period, including newborn care. This category requires you to understand normal physiological changes during pregnancy, recognize complications, manage labor and delivery, and provide evidence-based postpartum and newborn care.
Antepartum Care
Know the normal physiological changes of pregnancy by trimester and the expected discomforts and their management. High-risk pregnancy conditions heavily tested include preeclampsia, gestational diabetes, placenta previa, and placental abruption. For preeclampsia, know the classic triad of hypertension, proteinuria, and edema, along with the progression to eclampsia and HELLP syndrome. Understand magnesium sulfate administration, its therapeutic level of 4 to 7 mEq/L, and the antidote calcium gluconate.
Intrapartum Care
Labor and delivery questions focus on the stages of labor, fetal monitoring interpretation, and nursing interventions during labor. Know the four stages of labor and expected duration of each. Understand fetal heart rate patterns: baseline rate, variability, accelerations, and decelerations. Late decelerations indicate uteroplacental insufficiency and require immediate intervention. Postpartum and Newborn Care
Postpartum assessment uses the BUBBLE-HE framework: breasts, uterus, bladder, bowels, lochia, episiotomy, Homan sign, and emotions. Know the normal involution process and signs of complications like postpartum hemorrhage and infection. Newborn assessment includes APGAR scoring, gestational age assessment, and recognition of common newborn conditions. Breastfeeding education and support are frequently tested topics. Complications and Emergencies
Be prepared for questions on obstetric emergencies including shoulder dystocia, prolapsed cord, uterine rupture, and amniotic fluid embolism. Know the immediate nursing actions for each emergency. For prolapsed cord, the nurse should position the patient in Trendelenburg or knee-chest position and apply gentle upward pressure on the presenting part.
