Fluid and Electrolyte Imbalances
Fluid Volume Deficit (Dehydration): Assess for decreased skin turgor, dry mucous membranes, concentrated urine, tachycardia, hypotension, and increased BUN-to-creatinine ratio. Interventions include IV fluid replacement and monitoring intake and output. Fluid Volume Excess (Overload): Assess for edema, weight gain, crackles, distended neck veins, and dyspnea. Interventions include fluid restriction, diuretics, sodium restriction, and positioning in high Fowlers position.
Types of Shock
Hypovolemic shock results from fluid or blood loss. Assess for tachycardia, hypotension, cool clammy skin, and decreased urine output. Treatment includes IV fluids and blood products. Cardiogenic shock results from pump failure. Assess for jugular venous distension, crackles, and S3 heart sound. Treatment includes inotropes (dobutamine) and vasopressors. Distributive shock includes septic (vasodilation from infection), anaphylactic (allergic reaction), and neurogenic (spinal cord injury). Septic shock: administer antibiotics within one hour and fluid resuscitation. Anaphylactic shock: administer epinephrine immediately.
Emergency Response
For cardiac arrest, initiate CPR immediately with high-quality chest compressions (100-120 per minute, 2 inches deep). Apply AED as soon as available. Administer epinephrine every 3-5 minutes. For respiratory emergencies, open the airway using head-tilt chin-lift (jaw thrust if spinal injury is suspected). Provide supplemental oxygen and prepare for intubation if needed.
Acid-Base Imbalances
Use the ROME mnemonic: Respiratory Opposite (pH and CO2 move in opposite directions), Metabolic Equal (pH and HCO3 move in the same direction). Respiratory acidosis: pH below 7.35, CO2 above 45.
