Psychosocial Integrity: NCLEX Category Guide

Psychosocial Integrity accounts for approximately 6-12% of NCLEX-RN questions. This category tests your ability to promote and support emotional, mental, and social well-being of clients experiencing stressful events and mental health conditions.

Therapeutic Communication

Therapeutic communication techniques include open-ended questions, reflection, restating, clarification, silence, and summarizing. Non-therapeutic techniques to avoid include offering false reassurance, giving advice, asking why questions, changing the subject, and expressing approval or disapproval. Remember that the nurse role is to facilitate the patient expression of feelings, not to solve problems or share personal opinions. The correct answer on NCLEX is almost always the response that encourages the patient to explore their feelings further.

Crisis Intervention

A crisis is a time-limited event (typically 4-6 weeks) that overwhelms a person usual coping mechanisms. The nurse role in crisis intervention is to provide safety, assess for self-harm risk, facilitate coping, and connect with support resources. Always ask directly about suicidal ideation. Asking does not plant the idea. Assess lethality by evaluating plan specificity, access to means, and timeline. Remove access to lethal means and ensure continuous observation for high-risk patients.

Grief and Loss

Kubler-Ross stages of grief: Denial, Anger, Bargaining, Depression, Acceptance. These stages are not linear and not everyone experiences all stages. Support patients where they are without pushing them toward acceptance. Anticipatory grief occurs before an expected loss. Complicated grief involves prolonged, intense grief that impairs daily functioning and may require professional intervention. Cultural considerations are essential as grief expression varies significantly across cultures.

Mental Health Conditions

Know the nursing management for common conditions including major depressive disorder, bipolar disorder, schizophrenia, anxiety disorders, PTSD, eating disorders, and substance use disorders. Focus on safety interventions, medication management, and therapeutic communication approaches specific to each condition.
